Outside Agency Request
When requesting a certificate, please keep in mind the following:
In order to better assist you we ask that you:
- The registers and information contained in our records are the property of the Diocese of Camden.
- Requests of information from registers from government or corporate agencies (e.g. Social Security, insurance companies, etc.) should be accompanied by a signed letter from the person whose record it is (or from a legally qualified guardian), authorizing the release of information.
- Certificates are intended for internal church use, not civil purposes.
- Keep in mind there may be errors due to the word-of-mouth information given at the time the sacrament was received.
- Due to the sensitive nature of the information, certificates may not be sent by email or information given over the phone.
- We will mail a copy with the parish seal to the entity requesting the certificate. Or you may pick up the document in person.
- We may also fax a copy directly to the agency requesting the certificate.
In order to better assist you we ask that you:
- Please contact us in a timely manner. We ask that you give us at least 2 weeks prior to the date when you will need the certificate.
- Please provide a copy of your birth certificate or government issued ID.
- IF A MINOR: The birth certificate of the child.
- If you have the original certificate and need an updated copy (i.e. a new baptismal certificate for a marriage) you may mail or email a copy of the original certificate, and this will serve as proof of identity. A birth certificate or ID will not be required.
- Complete this request form: Sacramental Request and return it to us in person, by mail, or email.
