Sacramental Certificate Request
A certificate is an official document certifying that a particular individual has received a sacrament or funeral rites of the Church.
Each certificate is an exact duplicate of data entered in a sacramental register.

When requesting a certificate, please keep in mind the following:
- Sacramental Certificates are issued to the person listed on the record or the legal parent/guardian of a minor child or the power of attorney for a legal adult.
- They are issued to parishes for sacramental reasons or a Catholic Diocesan Tribunal for annulments.
- Certificates are intended for internal church use, not civil purposes.
- Due to the sensitive nature of the information, certificates may not be sent by email or information given over the phone.
- We will mail a copy with the parish seal to you or to the church/tribunal requesting the certificate. Or you may pick up the document in person.
- We may also fax a copy directly to the parish requesting the certificate.
In order to better assist you we ask that you:
- Please contact us in a timely manner. We ask that you give us at least 2 weeks prior to the date when you will need the certificate.
- Please provide a copy of your birth certificate or government issued ID.
- IF A MINOR: The birth certificate of the child.
- If you have the original certificate and need an updated copy (i.e. a new baptismal certificate for a marriage) you may mail or email a copy of the original certificate, and this will serve as proof of identity. A birth certificate or ID will not be required.
- Complete this request form: Sacramental Request and return it to us in person, by mail, or email.
